Rales or crackles in the lungs, an S3, and elevated jugular venous pressure

Frank acute pulmonary edema.

Cardiogenic shock or hypotension (measured as systolic blood pressure <90 mmHg), and evidence of peripheral vasoconstriction (oliguria, cyanosis or sweating)
